• 4 deductibles to choose from

• 100% or 80% choice of coverage after deductible is met

• $5 million lifetime maximum benefit

• Tax benefits up to $2900 per individual or $5800 per family

• Preventative care covered at 100% of first $300 per person per year 

• Rx drugs covered 100% after deductible

• Optional Dental benefits
