• A personal health plan with concierge level service and the benefits you'd expect from big employers

• 2 low deductibles to choose from

• $5 million lifetime maximum

• $35 Primary Doctor co-pay and $50 Specialist co-pay

• Preventative care benefits

• $15 Tier 1 generic Rx co-pay (co-pays for Rx Tier's 2-4 after $500 deductible)

• Optional benefits include Accident Rider,  Dental,  No Deductible Rx, and Lifetime Buy-up to $8 million
